The Osage Nation Health System Community Health Representatives (CHRs) program is dedicated to promoting a superior standard of community health, our initiative focuses on delivering top-notch health prevention, education, and information. By elevating awareness and advocating for healthier lifestyles, we aim to positively impact the well-being of Osage Nation community members.
TRANSPORTATION
The Community Health Representatives (CHR) program strives to support access to comprehensive health care. CHRs are well-trained, medically guided healthcare paraprofessionals. They act as liaisons and moderators between health, social, and community services to obtain access to care. Being from the communities they serve is one of the defining characteristics of CHRs and a critical factor in their effectiveness and ability to build trust. The trusting relationship enables the CHR to serve as an advocate and facilitator, addressing the healthcare and social support needs of individual Native Americans/Alaskan Natives and their families.
CHRs provide some transportation for routine medical appointments. Call for eligibility and availability.
CHILD PASSENGER SAFETY
In an effort to promote child passenger safety, CHR offers a car seat program. Families may contact the CHR office to schedule an appointment to have a car seat checked to ensure proper installation. Eligible families may receive a car seat at a reduced rate of $15.

AMERICAN HEART ASSOCIATION INSTRUCTOR-LED COURSES
Since 2008, the Osage Nation CHR program has been an American Heart Association Training Site under the Oklahoma State University Fire Service Training Center and provides Instructor-Led Training for the following:
Heartsaver® CPR AED is a video-based, instructor-led course that teaches adult, child, and infant CPR, AED, and choking relief. This course teaches skills using the AHA’s research-proven standards, which allow instructors to observe students, provide feedback, and guide their learning.
Basic Life Support (BLS). The Provider BLS Course teaches medical field participants to promptly recognize several life-threatening emergencies, perform high-quality chest compressions and ventilations, utilize an effective team approach, and provide early use of an AED. Students work with an AHA BLS Instructor to complete BLS skills practice and skills testing. Students also complete a written exam.

TAI CHI: MOVING FOR BETTER BALANCE
*All CHR Employees are certified American Heart Association (AHA) Instructors.
Tai Chi: Moving for Better Balance is an evidence-based, community-based falls-prevention program. Falls are the leading cause of fatal and non-fatal injuries in older adults.
Potential Health Benefits:
- Improves physical health, which leads to better functioning in daily activities
- Improves balance, posture, muscle strength, flexibility, and mobility
- Better mental health, such as increased self-esteem and less social isolation
- Improves musculoskeletal conditions
- Improves cardiovascular functions (reduces Hypertension)
- Improves sleep quality
- Enhances Stamina
- Increases energy level
