Eligibility
Our Osage Nation Health System Services are for:
- Federally recognized American Indian and Alaska Natives in the United States
- A non-IHS eligible female currently pregnant with an IHS eligible male’s child for the duration of her pregnancy (including initial pregnancy test). Along with the above information, submit your marriage license or notarized paternity affidavit, the child’s father’s picture ID, proof of pregnancy such as blood serum HCG test or ultrasound, and a signed Non-Beneficiary Acknowledgement form.
Required documentation for patients 19 years and older:
- Picture ID (Unexpired Driver’s License or State I.D or not needed if picture on Tribal Enrollment Card)
- Tribal Enrollment Card (or CDIB, Letter of Descendancy, and BIA card, will be considered DESCENDANTS)
- Social Security Card (not needed for in-house Medical care or if printed on Tribal Enrollment Card) If REFUSED – It could delay any future referral as other facilities do require them.
- Insurance Cards (Medicaid, Medicare, and/or private insurances, will need front and back of card scanned)
- Proof of Residency not need for in-house Medical care.
- Utility bill in patient’s names
- Lease/Rent agreement in patient’s name.
- If residing/boarding with another household, residency still applies.
- A notarized statement from head of household verifying residency to that effect must be submitted and along with utility bill or rent/lease agreement.
- If utility bill or rent/lease agreement is in spouse’s names, a a notarized statement, along with utility bill or rent/lease agreement. If REFUSED – It could delay any future referrals as other facilities do require them.
Required documentation for patients 18 years and younger:
- Picture ID (Unexpired Driver’s License or State I.D or not needed if picture on Tribal Enrollment Card) – will need parent or legal guardian’s
- Tribal Enrollment Card (or CDIB, Letter of Descendancy, and BIA card, will be considered DESCENDANTS) Children under 18 years of age may use parent’s
- Social Security Card (not needed for in-house Medical care or if printed on Tribal Enrollment Card) If REFUSED – It could delay any future referral as other facilities do require them.
- Insurance Cards (Medicaid, Medicare, and/or private insurances)
- Certified Birth Certificate (3 months and up)
- Guardianship or Placement Documents (if applicable)
