Welcome to the Primary Care Department at the Wahzhazhe Health Center, where we prioritize culturally sensitive and holistic healthcare to meet the unique needs of our community. Our department is committed to providing comprehensive and compassionate primary care services for individuals and families.
In our Primary Care Department, we focus on preventive care, health education, and disease management to empower individuals to take control of their health. Our services cover a wide range of medical needs, including routine check-ups, vaccinations, chronic disease management, and health screenings.

Services Provided
Direct services are available to all eligible Native Americans and referred care is allowable to those residing within the Osage Nation Reservation, Kay, Grant, Noble, Payne, Pawnee, and Garfield Counties. Osage Nation Health Services provides compassionate physician services to all ages. Services include the following:
Primary Care: Needing routine health and wellness services or care for known medical conditions.
Chronic Care: Needing ongoing care for a chronic disease. A disease or condition is chronic when it lasts a year or more, requires ongoing medical attention or limits the activities of daily life.
Acute Care: Needing attention by a physician within 72 hours, such as flu symptoms.
